The following information shall be required to accompany an application for a temporary permit:
(A) The name, address and phone number of the applicant on a completed application form supplied by the town;
(B) A description of the proposal, including a discussion of hours of operation, potential noise impacts, parking accommodation, impacts on adjacent property, any associated signs or lighting and provision for temporary utility services, as applicable;
(C) Appropriate filing fee amount; and
(D) Any other information deemed appropriate by the Administrator for complete review of the application.
