(a) Application for the license shall be made to the Director of Finance on forms to be prescribed by him, and shall state the name and address of the applicant, if individuals, and if a firm or partnership, the names, residences and occupations of the manager and each member of the firm or partnership, and if a corporation, the names of its officers and manager and such other information as the Director of Finance may require. The Director of Finance shall transmit such application for inspection and investigation to the Building Commissioner, Fire Chief and Police Chief. The Building Commissioner, Fire Chief and Police Chief each shall, after such inspection and investigation as they may deem necessary, make a report with their recommendation for approval or disapproval of such application. At the time the application is made there shall also be submitted a plan showing parking and exits from the game room. This plan shall be submitted by the Building Commissioner to the Planning Commission for their approval. If the application and the plan are approved by the Planning Commission, Building Commissioner, Fire Chief and Police Chief, the Finance Director shall issue a license on payment of the proper fee.
(b) The application for a license shall be rejected if the report submitted thereon shows that any person named in the application is not of good moral character or that the game room sought to be licensed does not comply with the health and fire regulations applicable thereto.
