§ 111.05  LICENSE APPLICATION REQUIREMENTS.
   (A)   Application for a license for a carnival or circus shall be filed with the Mayor on a form to be furnished by the Mayor and shall require the following information:
      (1)   Name of applicant;
      (2)   Home address of applicant;
      (3)   Name and address of the person or corporation operating the carnival or circus;
      (4)   All places where any such carnival or circus has operated during the past two months;
      (5)   A description of the circus or carnival activities which are to be presented; and
      (6)   A detailed list of rides or structures which are to be offered to the public, giving a list of all safety inspections and maintenance services performed on the same during the immediately preceding 30 days.
   (B)   Such application shall be made at least ten days before the license is granted.
   (C)   (1)   If the Mayor determines that, after an investigation, the applicant proposes to engage in the lawful conduct of a carnival or circus or proposes to have carnival or circus activities and the same do not, based upon past record, constitute a clear and present danger to the residents of the city, the Mayor shall issue a license to the applicant.
      (2)   Such license may be restricted by the Mayor to those activities, rides and structures which are free from defects and dangers constituting a peril to the health and safety of the public.
