§ 98.011 INFORMATION REQUIRED BY CITY.
   The city requires information from the arranging funeral home to include the deceased individual's name, lot identification, date of birth and death, date/time of burial and type of container. The city also requires that a licensed mortician certify or attest to the remains being placed in interment. This record will be recorded by the city and will constitute a permanent record.
