A permit shall be issued only upon a written application with the Clerk-Treasurer, signed by the person desiring to make the excavation, cut or bore. The application shall describe the following:
(A) The place where the excavation, cut or bore is proposed to be made.
(B) The purpose for which the excavation, cut or bore is to be made.
(C) The date the proposed excavation, cut or bore is to be started.
(D) The date the proposed excavation, cut or bore is to be refilled and completed.
