762.03 APPLICATIONS FOR PERMISSION.
   Any application presented to the City Manager for permission to make or conduct a public solicitation shall be presented at least ten days before the proposed date of the solicitation, and shall state:
   (a)   The name of the person, group of persons, firm, corporation, club, organization, association or institution, making the application;
   (b)   The names and addresses of the officers of the applicant, if there are officers, or three responsible persons affiliated with the applicant;
   (c)   The purpose for which the funds are to be collected and the use that is to be made of them;
   (d)   The names and addresses of all persons who will be active or otherwise take part in the solicitation;
   (e)   The proposed time of the solicitation; and
   (f)   The manner in which the solicitation is to be made.
