701.10 CONTENTS OF CERTIFICATES AND STUBS.

   Each certificate and the stub or other record thereof shall set forth the following items:

   (a)   Date issued;

   (b)   To whom issued;

   (c)   Purpose and authority granted;

   (d)   Premises or places to which applicable;

   (e)   Certificate number;

   (f)   Date of expiration or period for which license or permit is valid;

   (g)   Amount paid therefor;

   (h)   Amount of bond posted, if any;

   (i)   Place for City Treasurer to stamp "paid"; and

   (j)   Signature of issuing officer.
