(A) (1) The managers, partners, operators, or persons having supervisory control over the premises of the private club shall be identified upon the license.
(2) Each manager, partner, operator, or person having supervisory control over the premises of a private club shall make application to the Chief of Police as herein provided, and shall be approved by said Chief of Police as to his or her personal qualifications.
(B) An application for a change in or addition to the managers, partners, operators, or persons having supervisory control over the premises of a private club shall be accompanied by an application fee of $5.
