§ 34.03 HEALTH AND ACCIDENT INSURANCE.
   (A)   The village shall provide all qualified employees as set forth in any current ordinance with medical insurance, life and long term disability insurance. This insurance shall be under a group plan with an agency chosen by the municipality. The scope of this plan may be changed by action of Council.
   (B)   Only dependents who have been sired or legally adopted by the employee, or who are under sole legal custody of the employee or his or her spouse shall be eligible for coverage. The village may require the employee to produce adequate proof, including a birth certificate, the appropriate legal adoption or custody papers.
   (C)   Premium costs for full-time and eligible permanent part-time employees are shared by the employer and employee in amounts determined by the employer, which amounts are subject to change.
   (D)   An employee may opt out of medical insurance coverage by the municipality at any time during the year. However, an employee shall not be eligible to reinstate coverage except during the open enrollment period or within 30 days of a qualifying event, as defined by and pursuant to the policies of the municipality’s health care provider.
