5-1-15: ACCIDENTS, NONVEHICULAR:
   A.   Personal Injury: If you incur an injury in the line of duty, you are required to report the injury to the chief as soon as possible. The report shall contain the following information:
      1.   Name of injured employee.
      2.   Date and time of injury.
      3.   Brief description of accident.
      4.   Witnesses, if any.
   B.   Department Property: When departmental property (other than automotive) is lost or damaged, the officer in charge of the shift involved is to make a report to the fire chief as soon as possible. The report shall contain the following information:
      1.   Description of property.
      2.   Extent of damage.
      3.   Circumstances of the incident.
      4.   Opinion of the officer as to the responsibility for the loss or damage. (1987 Code)
