(A) The supplier of water shall deny or discontinue, after reasonable notice to the occupant thereof, the water service to any premises wherein:
(1) Any backflow prevention device required by this chapter is not installed, tested, and maintained in a manner acceptable to the supplier of water;
(2) If it is found that the backflow prevention device was removed or by-passed;
(3) If an unprotected cross-connection exists on the premises;
(4) If a low pressure cut off on a booster pump is not installed or maintained in good working order; or
(5) If the supplier of water or his or her authorized representative is denied entry to determine compliance with this chapter or any provision of the Ohio Administrative Code.
(B) Water service to such premises shall not be restored until the consumer has corrected or eliminated such conditions or defects in conformance with this chapter, and to the satisfaction of the supplier of water.
