Before any permit is issued by the village, an application shall be made on the forms as provided by the village and shall contain the following information:
(A) Name and address of applicant;
(B) Name of liability insurance carrier;
(C) Serial number, make, model, and description of the golf cart or utility terrain vehicle;
(D) Proof of insurance coverage specifically for the vehicle to be operated pursuant to the permit;
(E) Photocopy of the applicant’s current driver’s license; and
(F) Such other and further information as the village may require.
