§ 55.16 MEMBERSHIP RULES.
   The following rules are to be followed by all members and verification given upon request. The rules may be amended from time to time.
   (A)   Rule number one - alliance master record. Each member shall complete a record form to become part of the permanent files of the Alliance. This record shall be updated periodically but not less frequently than once a year. The record must contain statistical and general information pertaining to the evaluation of employee training and system safety.
   (B)   Rule number two - safety standards. It shall be mandatory that all members base their individual system safety plans and risk management programs on the current federal and state Pipeline Safety Acts, being 220 ILCS 20/1 et seq. Members are required to adopt and enforce NFPA-54 by ordinance or tariff.
   (C)   Rule number three - incident reporting. Every member shall make immediate telephone contact with a representative designated by the Alliance upon the occurrence of any incident, accident, or fire that might, in any way, possibly involve the gas utility. A telephone number for this purpose has been furnished to each member. (See Article 8.1 of this manual.) An immediate investigation will be initiated for all accidents.
   (D)   Rule number four - annual reports. Every member shall forward to the GUA office each year all of the following documents:
      (1)   Annual DOT Form RSPA F7100.1 (prior to DOT deadline date);
      (2)   Annual Form EIA-176 (prior to the EIA deadline date); and
      (3)   Annual Threat Summary from their DIMP Plan (prior to March 1 each year).
   (E)   Rule number five - member safety references/mandatory. The references in § 55.17 must be maintained by every member of the Alliance in current and useable form by one designated employee of the member system who shall also be responsible for making these known and available to all employees.
