809.16 MAPS, RECORDS AND REPORTS.
   (a)   A grantee shall annually provide to the City current maps of its existing and proposed installations, if any changes have occurred.
   (b)   A grantee shall annually file with the City a statement of revenue, along with the grantee's annual report, which includes the income statement and balance sheet only to the extent necessary for the City to carry out its regulatory responsibilities.
   (c)   A grantee shall submit such other reasonable information as may be requested by the City with respect to its property, revenues, expenses or operations, within the City, only to the extent necessary for the City to carry out its regulatory responsibilities.
   (d)   A grantee shall, upon request, furnish a current listing of its officers and those of any guarantor.
   (e)   An accurate and comprehensive file shall be kept by the grantee of all subscriber and user complaints regarding the cable system. A procedure shall be established by the grantee by the time of installation of the system to quickly and reasonably remedy complaints. Complete records of the grantee's actions in response to all complaints shall be kept. These files and records shall remain open to the public during normal business hours, subject to Federal law.
   (f)   A grantee shall submit such other information or reports in such form and at such times as the City may request to carry out regulatory responsibilities.
   (g)   A grantee shall allow the City to make inspections of any of the grantee's facilities and equipment at any time during normal business hours, upon forty-eight hours notice. A representative of the grantee shall be present.
   (h)   On or before 120 days after the end of a grantee's fiscal year, and each successive year during the term of the grantee's franchise, the grantee shall submit a written annual report to the City, including the following information:
      (1)   A summary of the previous year's (or in the case of the initial reporting year, the initial year's) activities and development of the cable system, including, but not limited to, services begun or discontinued, total number of subscribers and subscribers added or discontinued during the reporting year; and
      (2)   A list of stockholders holding ten percent or more of the voting interest in the grantee.
   (i)   Upon request, a grantee shall submit to the City copies of all decisions, correspondence and actions by any Federal, State and local courts, regulatory agencies and other governmental bodies, relating to its cable television operations within the franchise area.
   (j)   A grantee shall make available to the City such other information or reports pertinent to enforcing the terms of the franchise, in such forms and at such times, as the City may reasonably request upon reasonable advance written notice.
   (k)   A grantee shall allow the City to make inspections of any of the grantee's facilities and equipment at any time upon one business day's prior notice.
   (l)   The refusal of the grantee to file any of the records or reports required under this section to be made upon request in writing shall be deemed a material breach of the franchise and shall subject the grantee to all penalties and remedies, legal or equitable, which are available under the franchise or this chapter.
   (m)   Any materially false or misleading statement or representation knowingly made by the grantee in any report required under the franchise or in any notices to subscribers and users shall be deemed a material breach of the franchise and shall subject the grantee to all penalties and remedies, legal or equitable, which are available.
