937.16 PERMIT SYSTEM.
   (a)    A permit form shall be filed for each new sewer tap and/or sewer line extension.
   (b)    Permits shall be in three classes:
      (1)    Form 1 - Residential or Commercial.
      (2)    Form 2 - Industrial.
      (3)    Form 3 - Sewer line extensions, including manholes, pumping stations, etc.
