11-11-3: APPLICATION FOR PERMIT:
An application for a demolition permit, signed and verified by the owner (the "permittee") of the property on which the building or structures to be demolished are located, or the owner's agent, shall be filed with the Building Official and shall include the following information:
   A.   The street address of the building or structure to be demolished.
   B.   A plan for demolition and a schedule of time to complete the demolition activities.
   C.   The name and address of the contractor to perform the demolition work, including a copy of the contractor's license, as applicable.
   D.   Evidence the permittee has satisfied the insurance requirements of this chapter.
   E.   The name and address of the landfill at that the permittee will use to dispose of Unsuitable Materials, as defined herein.
   F.   If the building or structure is infested with rodents or insects, the applicant shall supply documentation that such infestation has been eliminated.
