§ 122.21 INSPECTIONS.
   (A)   The Director of Health, or his authorized agent(s), shall be permitted to enter, during normal operating hours, any portion of any barbershop or hairdressing and/or cosmetology shop/salon for the purposes of making inspections to determine compliance with this chapter and the Public Health Code of the state.
   (B)   At least once a year, the Director of Health or authorized agent, shall inspect each barbershop, hairdressing and/or cosmetology shop and shall make as many additional inspections as are necessary for the enforcement of this chapter and the Public Health Code of the state. Any shop offering pedicures as a service shall be inspected a minimum of twice a year.
   (C)   Corrective actions shall be completed and reinspections shall be conducted in a time period not to exceed two weeks if there are one or more major inspection items in violation.
   (D)   Major inspection items are:
      (1)   Potential exposure to infectious, pathogenic or toxic agents;
      (2)   Inadequate water supply; inadequate sewage disposal; unapproved means of sewage disposal;
      (3)   An unlicensed individual is performing procedures required by licensure;
      (4)   Lack of an approved disinfection; use of an unapproved disinfectant; improper use of an approved disinfectant;
      (5)   In residence shop not completely separate from living/sleeping quarters;
      (6)   Toilets, washbasins and utility sinks inadequate, inconvenient, inaccessible, improperly designed and/or installed;
      (7)   Cross connections or potential back siphonage or backflow; and
      (8)   Equipment and utensils used on customers not cleaned and/or disinfected after each customer.
