§ 152.03 FORM OF APPLICATION.
   (A)   Every application for an improvement location permit shall be accompanied by a site plan, drawn to scale, showing:
      (1)   The location of the structure;
      (2)   The improvement or use to be altered, changed, placed, erected or located;
      (3)   The dimensions of the lot to be improved;
      (4)   The size of yards and open spaces;
      (5)   Existing and proposed streets and alleys adjoining or within the lot; and
      (6)   The manner in which the location is to be improved.
   (B)   The fee that shall accompany an application for an improvement location permit shall be determined from time-to-time by the Building Commissioner in his or her discretion and any modification in the fee so charged shall, at the request of the Building Commissioner, be reviewed from time-to-time by the Washington Common Council who may by appropriate resolution approve or reject any modification so proposed by the Building Commissioner.
