Applicants must go to the Daviess County Chamber of Commerce to obtain a criminal background check and an identification paper, then must complete and return to the Clerk-Treasurer's office, before the applicant shall be authorized to do business, an application furnished by the Clerk-Treasurer's office which shall require the following information:
(A) Name, permanent address, and telephone number of business being represented;
(B) Nature of business/description of product;
(C) Name, address, telephone number, and photocopy of drivers license for each person representing the business;
(D) Make, model, color, and license plate number of any vehicle to be used by the personnel representing the business;
(E) Standing or rating with the Better Business Bureau;
(F) Statement of whether the applicant has been cited for an ordinance violation related to the applicant's business endeavors within the last 3 years; nature of the offense and place of conviction.
