Contractors must complete an application for certification showing proof of liability insurance, current state license (if applicable), letter of credit equal to the amount of the job, and sign a document stating that they have read Chapter 97 in the Code of Ordinances and are willing to comply with all requirements and that they are liable for any damage to public or private property. Failure to meet these requirements could result in a fine or revoking of certification. Applicant must meet requirements set by the Street Commissioner, Building Commissioner, Code Enforcement Officer and City Engineer. These requirements will be approved by the Board of Public Works and Safety.
