(A) The license fee for each coin-operated amusement device shall be $25 per year.
(B) Prior to issuing any license for any coin-operated amusement device, the City Manager or his designee shall require an application to be filed. Such application shall contain information sufficient to identify the owner, identify the coin-operated amusement device, its location, the owner’s insurance and any other information the City Manager deems necessary to protect the health and welfare of the residents of the city.
