All individuals making an application for a public safety sensitive position shall complete an application that shall contain the following:
(A) Full name, place and date of birth and the present residence address and residence addresses for the past five years.
(B) Whether the applicant has ever used or been known by a name other than his or her present name, and if so, what such name or names were and information concerning dates and places where such names were used.
(C) Whether the applicant has been convicted of any crime. If so, the applicant shall furnish information as to the date, place and offense resulting in the conviction.
