A. Any person who witnesses or has personal knowledge that an act or acts made unlawful by this chapter have been committed may sign a complaint against the alleged violator.
B. Any police officer, animal control officer or code enforcement officer who is employed by the city is authorized to issue a summons and complaint when the officer personally observes a violation of this chapter.
C. The complainant must provide a sworn complaint to the officer receiving the complaint containing the following information:
1. Name, address and telephone number of the complainant and other witnesses to the incident;
2. Date, time and location of the incident;
3. Description of the animal;
4. Name, address and telephone number (if known) of the animal owner;
5. A statement that the animal attacked the complainant or some other person or animal as witnessed by the complainant, or such facts as warrant a finding that the animal is vicious, dangerous or diseased;
6. Other facts and circumstances of the incident. (Ord. 743, 1993)
