A. An application for an attendant's license shall be made upon blank forms furnished by the city and shall contain:
1. The applicant's full name, present resident address and any other residence address during the past year;
2. Age, date of birth, weight, height, color of eyes and hair, and driver's license number;
3. a. A statement of all felonies and misdemeanors (including moving traffic violations and excluding parking violations) for which the applicant has been convicted within the last five (5) years immediately preceding application for the license,
b. The applicant applying for the position of wrecker driver shall provide a copy of his/her driving record from the Oklahoma state department of public safety, and a copy of his/her criminal record from the Oklahoma state bureau of investigation (OSBI);
4. A statement of past experience as a wrecker attendant, including the name and address of each employment as a wrecker attendant within the five (5) years immediately preceding application for the license;
5. All applications shall be signed by the applicant under penalty of perjury.
B. Applications for an attendant's license shall be submitted to the city clerk together with the payment of three dollars ($3.00) for a processing fee for each application. (Ord. 870 §1, 1998: Ord. 740 §1, 1993)
