Application for permits required by this chapter shall be filed with the city clerk on forms to be provided by the city and shall contain the following information:
A. The name, home address and actual business address of the applicant;
B. If the applicant is a partnership, the name, residence address and telephone number of each partner. If the applicant is a corporation, the same information on each director, officer and the general manager;
C. A financial statement prepared by a certified public accountant or on form based on generally accepted accounting principles;
D. List of any unpaid judgments against the applicant or its principals;
E. A statement of the experience of the applicant and its principals in providing wrecker service;
F. The location, number, description and hourly availability of the wreckers owned or operated by the applicant;
G. A description for the applicants enclosed facility to be used for the storage and protection of vehicles which are impounded;
H. The names and addresses of all proposed attendants;
I. A copy of the current class A wrecker operations license issued to the applicant by the state of Oklahoma;
J. A certificate of insurance establishing insurance requirements of this chapter;
K. Such other information as the city clerk, police chief or city council shall deem necessary. (Ord. 740 §1, 1993)
