Each permit for making any such opening shall be limited to a single project and shall be issued only to, or in the name of, the property owner requesting such permit. Such permit shall be issued by the Mayor and the Clerk and the fee therefor shall be as set forth in Chapter 208 of the Administration Code (the General Fee Schedule). Application for such permit shall be made on a form approved by the Mayor and shall show the exact location of the proposed opening, the kind of paving, the area, depth and estimated duration of such opening and such other pertinent information as may be requested.
