Permits required by Section 864.01 may be obtained by making application therefor to the Mayor upon such forms as he or she may prescribe. Such application shall be accompanied by a nonrefundable fee as set forth in Chapter 208 of the Administration Code (the General Fee Schedule). A separate application and fee shall be filed and paid for each well contemplated.
