The following information shall be submitted as part of a request for a conditional use permit to develop a multi-family or two-family development;
(A) A boundary survey showing the total acreage, present zoning classification(s), date and north arrow;
(B) The owner’s name, address and the tax parcel numbers of all adjoining properties;
(C) All existing easements, rights-of-way and all yard requirements for the zoning district;
(D) Proposed layout of the site and all structures. This shall include the number of units, number of stories of each building, and location of all doorways. Trash collection facilities as well as any common areas and recreation areas shall also be shown;
(E) Traffic, parking and circulation plans, showing the proposed locations and arrangement of parking spaces and access points to adjacent streets;
(F) Proposed screening and buffers, including walls, fences or planted areas, as well as any natural features to be retained on the site;
(G) Delineation of areas within a designated flood hazard area;
(H) Proposed number and location of signs;
(I) Proposed phasing, if any, and approximate completion time of the project;
(J) Approved driveway permit from the State Department of Transportation for developments that access state maintained roads and highways; and
(K) Certifications that an approved water and sewer system can be provided to service the project.
