The sponsoring exempt organization shall apply, in writing, to the town for a permit to conduct the requested activity on a form and in the manner so as to enable the town to know who is sponsoring the activity; the full scope of the intended activity; and the names, addresses and telephone numbers of all individuals within the sponsoring organization who are to supervise the activity and be answerable for the conduct of the activity; with each application, the sponsoring exempt organization shall provide the town with a copy or the original of a determination letter from the Internal Revenue Service or State Department of Revenue that indicates that the organization is an exempt organization.
