§ 155.115 NOTICE OF APPEAL.
   A notice of appeal to the City Council shall contain the following information:
   (A)   The date and heading of the decision from which the appeal is taken;
   (B)   A statement whether the appeal is taken upon matters of law, or upon matters of fact, or both;
   (C)   A statement of the issues that the appellant asserts on appeal; and
   (D)   The name, address, and signature of the appellant.
