All hearings upon an application for a certificate of appropriateness shall be electronically recorded. The record of a hearing before the Commission shall include:
(A) The application for a certificate of appropriateness;
(B) Evidence received or considered;
(C) A statement of matters officially noted;
(D) Questions and offers of proof, objections, and rulings thereon;
(E) Any proposed findings and exceptions; and
(F) Any decisions, findings of fact, and conclusions of law.
