(A) Generally. All applications for a license to practice massage shall be accompanied by a medical certificate from a physician duly licensed to practice medicine in the state stating that the applicant has no communicable disease. All initial applications shall be accompanied by a non-return able investigation fee. Applications shall contain such other information as the Council may, from time to time, require. All applicants shall be at least 18 years of age.
(B) Insurance requirements. All licensees shall at all times have a valid certificate of insurance issued by an insurance company licensed to do business in the state with minimum liability coverage of $500,000 for each claim and $1,000,000 for each aggregate claim. Said certificate of insurance shall be filed with the city as a condition of approval of the license by the City Council, with the city listed as an additional insured.
