(A) Application form. Each application for a permit required by this subchapter shall be made at City Hall upon a form as may be prescribed by the city and shall be executed by the applicant.
(B) Information.
(1) Each application for a permit required by the provisions of this subchapter shall include the following:
(a) Date of application;
(b) Name and address of applicant;
(c) Nature of the applicant’s physical handicap, if any;
(d) Model name, make, year and serial number of the motorized golf cart;
(e) Current valid driver’s license;
(f) Insurance company policy number and liability limits (insurance must be in the applicant’s name);
(g) Permit fee in an amount that may be prescribed from time to time by resolution of the City Council; and
(h) Other information as may be required.
(2) The applicant must provide all information required above prior to being issued a permit.
(C) False statements. It is unlawful for any person to intentionally make any false statement or to intentionally omit any required information in the completion of any application form. Any false statement or omission shall render the application, or any permit issued pursuant thereto, invalid and of no effect.
