(A) Application for the permit shall be presented to the Township Clerk in writing, setting forth the portion of the street, avenue, highway, road or land to be used, encumbered or obstructed as aforesaid or the portion of the curbing to be removed, the purpose or use to which the same shall be put and the period of time during which it shall be maintained, the size of the excavation, the date when the excavation is to be made and the probable time required and the nature of the pavement, curbing or surface to be broken, which application shall also set forth the name of the person requiring the same and shall be accompanied by a sketch showing the details thereof. (B) No opening of any kind shall be made and no curbing shall be removed without obtaining a permit from the Township Clerk. The required fees shall be established by the Township committee annually. All fees shall be paid at the time of the issuance of a permit for each transverse opening or curb removal. After the street, roadway or curb has been completed or restored to its original condition, the recipient of the permit shall so notify the Clerk. If an inspection of the opening indicates that the roadway has been restored in a satisfactory manner or that the curb opening has been completed in a proper manner, a refund less $10 shall be made on curb removals. If the roadway is not restored in a satisfactory manner, the entire amount of the fee shall be forfeited as liquidated damages.
(C) A fee in an amount set by Township Committee shall be paid for the issuing of a permit for each longitudinal opening. A refund will be made upon the satisfactory restoration of the roadway surface. At the option of the Township, an inspection charge may be made in an amount set by Township Committee.
(D) All fees shall be paid to the Township Clerk either in cash or by certified check drawn to the order of the Township.
