The completed application form furnished by the Township Clerk, which, when signed by the Township Clerk or Deputy Clerk, shall become the license for such dog and/or cat, shall state the breed, sex, age, color and markings of the dog or cat for which the license and registration are sought, whether the dog and/or cat is of a longhaired or shorthaired variety, the name, state and post office address of the owner and the person who shall keep or harbor such dog and/or cat.
