If on inspection the taxicab is found to be of lawful construction and in proper condition in accordance with the provisions of this chapter, and upon payment of the fees set forth in § 117.03(B) when so licensed there shall be delivered to the owner, ln addition to the license, a card of the size and form prescribed by the City Clerk-Treasurer. This card shall contain the official city license number of the taxicab company and a vehicle number for that particular taxicab, together with the date of inspection of the taxicab, and a statement to the effect that in case of any complaint the Police Department shall be notified, giving both the state license and vehicle ID numbers. The card shall be signed by the City Clerk-Treasurer and shall contain blank spaces on which an entry may be made of the date of every inspection of the vehicle by the Inspector designated. The license card shall be of distinctly different color each year and shall contain the same of the person owning the taxicab and the state license numbers issued therefore. On the payment of $2.50 to the City Clerk-Treasurer and furnishing proof by affidavit of the loss or destruction of the license or the card, a duplicate thereof shall immediately be issued to the licensee with the notation thereon that it is a duplicate.
