   (A)   Current Indiana business license;
   (B)   Current Knox County Health Inspection;
   (C)   Approval from the Board of Works if you wish to operate in the public right-of-way;
   (D)   Proof of independent safety inspection of all vehicles used in the business; and
   (E)   Proof of indemnity insurance.
