A permit shall be obtained from the Director prior to installation and connection of private sewage disposal facilities, residential and commercial building sewers and industrial sewers. A fee shall be charged for each permit as follows:
(A) For installation of residential disposal facilities, $25.
(B) For installation of residential building sewers, $500 per each dwelling unit.
(C) For installation of commercial building sewers, $350.
(D) For installation of industrial sewers, $400.
