Evaluations will be completed annually in January on all permanent non-probationary employees using a "Performance Evaluation for Non-Probationary Employees" form available at City Hall. Employees will be evaluated by department heads; department heads will be evaluated by their respective committee. Upon completion, forms will be submitted to the Office Manager for inclusion in the employee's personnel file and for review by the Mayor and City Council.
