§ 37.065  OTHER BENEFITS.
   (A)   Medical benefits.
      (1)   The city provides a full-time employee and his or her dependents with medical insurance as set forth in the governing medical insurance policy at a cost to be determined from time to time by the Council. Under the city's medical insurance policy, medical benefits are available on the first day of the employee's employment. Employees terminating employment with the city are entitled to participate in the city's medical insurance program at their own cost as set forth in the city's governing medical insurance policy, and by the COBRA laws. The city's medical insurance program is subject to change without notice.
      (2)   Life and disability insurance will be provided by the city for the employee, after the employee has been employed for 90 days.
      (3)   The city will continue to pay the city portion of the premium for those employees who are on an approved medical/injury leave with pay. Employees on leave without pay are responsible for payment of 100% of premium due in order to continue their coverage.
      (4)   The city may provide an opt-out cash benefit for employees who chose not to enroll in the medical insurance plan.
   (B)   Retirement benefits.  All permanent employees who are eligible are required to join the Illinois Municipal Retirement Fund (IMRF). Four and one-half percent of an employee's taxable wages will be withheld from each paycheck for contribution to the fund. This plan is governed solely by IMRF.
   (C)   Clothing.
      (1)   The city will provide shirts, steel-toed boots, gloves, and rain gear used by full-time employees, other than police officers, as needed for the performance of their duties.
      (2)   The Chief of Police and police officers are entitled to replacement uniform items purchased by the city in an amount up to $400 each year, beginning with the anniversary of the police officer's first full year of employment.
      (3)   All other non-police employees are entitled to a replacement uniform amount of $400 each year.
      (4)   For each newly-hired police officer, the city will purchase, from vendors selected by the Chief of Police, the uniform items set forth in Appendix A to this Personnel Code Ordinance. A newly-hired police officer will, within 10 days of being hired, provide, at his or her own expense, the uniform items set forth in Appendix B to this Personnel Code Ordinance. Upon termination, a police officer is expected to return to the city all items purchased by the city.
