(A) The following classes of officers and employees of the city and city agencies shall file an annual statement of financial interests with the Board of Ethics:
(1) Elected city officials.
(2) Candidates for elective city offices.
(3) City appointed members of the Planning and Zoning Commission, Board of Adjustment, and Economic Development Authority.
(4) Police Chief, Fire Chief, City Clerk/Treasurer/Tax Administrator, and Public Works Director.
(B) The City Clerk will deliver the financial disclosure statements to those specified in divisions (A)(1), (3) and (4) by December 23 of each year. The City Clerk will deliver to candidates for elective city offices within ten (10) business days of the last day to officially file for office.
(C) Failure of the City Clerk to disseminate the forms does not waive the responsibility of the filer to file the form by the required deadline.
