878.09 ISSUANCE OF LICENSES; CHANGE IN INFORMATION PROVIDED IN APPLICATION.
   (a)   All applications properly received by the Mayor shall be forwarded within seven days to the Director of Public Service and the Building Inspector for their review and possible approval.
 
   (b)   The Director of Public Service or the Building Inspector, as appropriate, shall determine that the following prerequisites to the issuance of a license have been established:
      (1)   The owner(s) and/or operator(s) have not been convicted within the previous five years of a felony, a misdemeanor involving minors, a misdemeanor involving moral turpitude or a crime of violence, or a misdemeanor involving gambling activity, controlled substances, or alcoholic beverages;
      (2)   A license properly issued to the applicant has not been revoked or forfeited within the previous five years;
      (3)   The location, structure, placement, wiring, and connections proposed for the sweepstakes terminal devices covered by the application are suitable under the requirements of the Building Code, the Zoning Code, and this chapter, and all required inspection fees have been paid to the City; and
      (4)   The building or premises in which the sweepstakes terminal device facility is located, and the location of the facility, is in compliance with all requirements of the Building Code, the Zoning Code (including placement in an I-2 Heavy Industrial District and the issuance of a conditional use permit), and this chapter, and all required inspection fees have been paid to the City.
 
   (c)   The Director of Public Service and the Building Inspector shall endorse on each application their approval or disapproval, with reasons for any disapproval, and return the same to the Mayor within thirty days of receiving the application. Within fourteen days of approval or disapproval by the Director of Public Service or the Building Inspector, and payment of the license fees, the Mayor shall either approve or deny the license pursuant to this chapter.
 
   (d)   Each owner or operator to whom a license has been issued must provide the Mayor with a written update when any of the information provided in the license application changes.
