(a) One sweepstakes device terminal facility license shall be issued for each sweepstakes device terminal facility at each individual premise, and shall specify the name and address of the licensee, and the manufacturer, model number, and serial number of each sweepstakes terminal device located at the facility for which it was issued.
(b) Change(s) or variation(s) to the sweepstakes terminal device facility license as originally issued, including without limitation any change in the number (whether an increase or decrease) of sweepstakes terminal devices in operation at a sweepstakes terminal device facility, shall invalidate the facility license. The owner or operator of the facility must obtain a new sweepstakes terminal device facility license from the City of Vermilion within seven calendar days of any change or variation to the facility license.
(c) A sweepstakes terminal device facility license shall be invalidated if the location within the facility of any sweepstakes terminal device, as shown on the floor plan submitted with the license application pursuant to Section 878.08(e)(10), is changed, unless the owner or operator provides the City with a revised floor plan showing the new location of all sweepstakes terminal devices in the facility within seven calendar days after any device is moved.
