(a) The fee for each City sweepstakes terminal device license shall be thirty dollars ($30.00) per device per month, paid biannually in advance as described in Section 878.05(a).
(b) The fee to file with the City a certified copy of the annual certificate of registration obtained from the Ohio Attorney General, pursuant to R.C. § 2915.02(F), and/or a certified copy of the annual certificate of compliance obtained from the Ohio Attorney General, pursuant to R.C. § 2915.02(G), shall be fifteen dollars ($15.00) for each filing.
(c) The fee for a City sweepstakes terminal device facility license shall be two thousand five hundred dollars ($2,500), every six months.
(d) The fee for a license purchased after January 1 or July 1 for commencement of operation after January 1 or July 1, as applicable, shall not be prorated.
(e) The fee for a replacement City license shall be fifteen dollars ($15.00).
(f) All City license fees and/or filing fees required under this chapter shall be filed with the application for the City license and/or document to be filed.
(g) In the event a City application for a license is denied under this chapter, one-half of the license fee(s), less any costs incurred for reports and investigations, shall be returned to the applicant.
(h) No portion of a license or application fee shall be returned to the owner or operator should a City or state license be suspended or revoked, or if the owner or operator otherwise ceases operation of the sweepstakes terminal device or devices and/or of the sweepstakes terminal device facility.
(i) Fees for filing documents are non-refundable.
