Any franchise awarded hereunder and the rights, privileges, and authority granted thereby shall take effect and be in force from and after the thirtieth day following the award thereof provided that within 30 days from the day of the award the grantee shall file with the city the following:
(A) A notarized statement by the grantee of unconditional acceptance of the franchise; and
(B) A certificate of insurance as set forth in § 111.09 (D).
