All applications for permanent driveways over fourteen feet in width shall be accompanied by a plat or print drawn to scale showing the location in the street, sidewalk or public place, where the license or privilege is desired, and also showing street lines, curb lines and the dimensions of the portion of the street, sidewalk or public place to be occupied. The plat or print shall also show the location of all poles, fire hydrants, trees, manholes or any other publicly or privately owned structures.
