SECTION 9-3.   FORM OF PETITIONS.
   All petition papers presented to Council relating to either initiative or referendum shall be uniform in size and character, and shall be substantially in the same format as provided by the Clerk of the Council. Initiative petitions shall contain the full text of the proposed ordinance. A certified copy of the Initiative or Referendum Petition shall be submitted to the Clerk of Council before circulation of same for signatures.
   All petition papers shall contain the names and addresses of five or more electors who shall be regarded as filing the petition and shall constitute a committee of the petitioners. All names shall be signed in ink and shall show the petitioners' place of residence.
   There shall be placed upon each separate petition paper an affidavit of the circulator thereof that he (or she) only, personally circulated the paper, and that each signature thereon was signed in his (or her) presence.
