(a) Submittals shall be made to the Village Engineer.
(b) The following design, preparations, and qualifications information shall be submitted with a minimum of two weeks prior to start of blasting, unless otherwise noted.
(1) The blast design as specified for each surface excavation, shaft or tunnel heading; resubmit if the design is changed in the course of the work; all blast plans shall be signed and approved by the Blaster-In-Charge. Included in the blast design shall be the design for the blast test holes including the number, location, and the weight of explosive in each blast test hole.
(2) Blasting safety plan, for engineer's information only.
(3) Form for recording blast report.
(4) Description of instrumentation to be used to monitor effects of blast, certificates of calibration for all calibrated components including geophones, microphones, and recording units, and a typical blast monitoring record.
(5) Manufacturer's literature of proposed vibration and air-pressure monitoring equipment.
(6) Qualifications of the Vibration Monitoring Specialist, including resumes and description of applicable experience.
(7) Qualifications of the Blaster-In-Charge, including a detailed description of education, training, and project experience within the specified time period; a minimum of three references with phone numbers; a copy of the Blaster's state license and all other required licenses and permits.
(8) A copy of all permits and notifications required by law for transportation, storage, and use of explosives, at least seven calendar days prior to start of blasting.
(9) A list of local officials, emergency service providers, and property owners notified of blasting program, and copies of written correspondence with them, one week prior to start of blasting.
(10) The contractor shall furnish the engineer with a list of those parties notified in accordance herewith prior to the start of such blasting. List shall include names, addresses and telephone numbers.
(11) The permission agreement signed by the property owner agreeing to allow the contractor to proceed with the blasting.
(c) The blasting plan submittals are for quality control and record keeping purposes. Review of the blast plan by the engineer shall not relieve the contractor of his responsibility for accuracy and adequacy of the plan when implemented in the field.
