Anyone wishing to have a building or structure designated as architecturally or historically significant shall file an application with the Planning and Environmental Commission, which application shall be made upon a form provided by the Administrator which shall contain the following information:
(A) The name and address of the owner or applicant;
(B) A legal description, street address or other identifying data concerning this site;
(C) A picture of the building or structure;
(D) A statement giving the age of the proposed building or structure;
(E) A brief description as to why the applicant considers the building or structure to have historical or architectural significance; and
(F) Such additional material as the Administrator may request or the applicant may submit pertinent to the application and to the findings prerequisite to the issuance of a designation.
