(a) The Village shall obtain insurance for each full-time employee who requests such coverage. This insurance shall consist of a minimum of medical insurance, dental insurance, prescription services and life insurance for the amount of ten thousand dollars ($10,000). The provider of such insurances shall be determined by the Village.

(b) The Village shall pay the following dollar amounts toward the monthly premiums for benefits obtained under subsection (a):
Single coverage | Village pays | $900.13 |
Married coverage | Village pays | $1,800.26 |
Family coverage | Village pays | $2,790.39 |
Single/children coverage | Village pays | $1,665.24 |
The remainder of the premium for such benefit shall be paid by the employee.

(c) Any employee requesting double or family coverage shall be first required to prove that such coverage is not available under another plan that is already being provided to one or more of the additional person or persons requesting coverage under the Village plan. If such coverage is available, the Village shall not be required to provide the additional double or family coverage.

(d) The Village shall pay up to one hundred dollars ($100.00) per month as reimbursement for prescription costs actually incurred by each surviving spouse of an employee who died while in active service to the Village and while covered under double or family Village health insurance. The benefits obtained under this section shall be payable only during the first year following the death of the qualifying employee.
